Maintain Monthly Contact
Stay in regular contact with your assigned Care Coordinator through phone, virtual, or in-person meetings.
Know Your Care Coordinator
Keep your Care Coordinator's contact information available and respond to communication.
Request Amazing Home Care Providers
Inform your Care Coordinator that you would like Amazing Home Care Providers to be your Behavioral Health Respite provider.
Verify Your Care Plan
Ensure Behavioral Health Respite services and Amazing Home Care Providers are included in your child's Care Plan.
Complete Quarterly CANS Assessments
Participate in required CANS assessments every 90 days to help maintain eligibility.
Participate in Care Planning
Attend meetings, complete requested documentation, and remain involved in your child's care planning process.
Report Important Changes
Notify us promptly of changes to: Contact information, Medicaid coverage, Guardianship, Placement, Hospitalization, Other important updates
