About the Respite Program
Our Behavioral Health Respite Program is designed for youth ages 0 to 20. This program provides parents and guardians with a much-needed break while their children participate in safe, structured, and engaging community-based activities.
Our goal is to support families while ensuring children are cared for in a positive and supervised environment.

- This is a Medicaid-based program
- A CANS assessment is required to determine eligibility
- Completing forms and assessments does not guarantee eligibility
- All information submitted is strictly confidential and safeguarded
- We do not share information with any outside entity
How to Enroll

Both forms must be completed by a parent or guardian.

Approval & Begin Services

Once approved, services will begin with support from our care team.
Eligibility is not guaranteed and is based on behavioral health needs.

Complete Your Enrollment Forms
Both forms are required to begin services.
To move forward with enrollment, parents or guardians must complete the Consent for Services form and the Release of Information (ROI) form.
Enrollment cannot be processed until both forms are submitted.
